Global Collaboration: East - West Exchange to Learn from Each Other

The discussion explores how differences in diabetes phenotypes between Europe and East Asia are helping researchers better understand the disease, improve treatments and develop new scientific collaborations across continents.

Professor Yabe also discusses the exciting progress being made in regenerative medicine, including clinical trials using iPS cell-derived islet cells for people living with type 1 diabetes. He explains how this approach could help overcome donor shortages and bring scalable cell therapies closer to clinical practice.

00:00:13: You're watching ESDTV Now here in Vienna.

00:00:17: this is right in the centre of Europe And the clue that this is a European, organisation of course in their name.

00:00:25: The European Association for the Study of Diabetes.

00:00:29: But increasingly EASD has become global event and we were wondering what those people who come from very far from Europe think about EASDS on why it's value to them?

00:00:42: I'm delighted welcome our studio today Daisuki Yabe from Kyoto in Japan.

00:00:48: You're very welcome, thank you so much for coming.

00:00:51: and

00:00:52: why do you come to EAST?

00:00:56: So it's almost fifteen years ago that was my first EAST.

00:01:04: that happened at Vienna.

00:01:07: From that point, I'm very fond of coming and attending the ESD.

00:01:13: Presumably because the EASD always talk about differences in similarity to diabetes phenotype between East and West.

00:01:27: And East means Asia.

00:01:30: so in ESD we often times exchange ideas phenotype or diabetes treatment, not only the Europe but also in Asia and Japan.

00:01:45: So I like that kind of exchanging ideas similarity differences.

00:01:50: so thats why Im being here

00:01:52: And we have a lot to learn from each other.

00:01:55: What's the big difference you see between Diabetes in Europe and Diabetes In Japan?

00:02:03: From my experience with the previous discussions was a colleague in EASD, where we found let's say that diabetes phenotype is often times obesity and then very high insulin resistance.

00:02:19: Yet especially East Asia like Japan or Korea-China our diabetics are not necessarily obese and the phenotype is primarily characterized by beta cell dysfunctions.

00:02:35: So those kind of differences are very much shared with ESD, ASD, Asian Association for the Study of Diabetes.

00:02:46: And actually exchanging this idea will give us hints on further research how to improve diabetes care in Asia.

00:02:57: Yes, it gives you huge sort of cues for new research.

00:03:02: Right and also the collaboration with that investigator in Europe is very beneficial to us in terms of doing the right research design or writing good papers, and especially disseminating our ideas not only Asia but also globally.

00:03:30: So I think that ESD colleagues always helps us to do a right science like implementation of diabetes care in Asia.

00:03:39: Now, you work particularly in stem cells.

00:03:42: Yes

00:03:42: Tell me a bit about that because that's one of the most exciting areas.

00:03:47: Thank You very much.

00:03:49: at the moment and we're seeing suddenly I've been reporting on stem cells for long time And it'll always be what are those things?

00:03:57: It is going to happen soon and never has.

00:04:00: How close do think we are To Islet transplants using stem cells.

00:04:06: Thank you very much.

00:04:07: So the eyelid transplantation has been conducted as a key therapeutic approach for people with type one diabetes, however in Japan or Asia that donor of the eyelid is pretty much limited and we need to have some approach to circumvent shortage.

00:04:36: So we actually went to the IPS cells, induced pluripotent cells.

00:04:43: And that's... Actually our colleagues in Kyoto University spent more than ten years to develop their IPS-derived islet cells and actually we have started a clinical trials transplanting the IPS derived islets cells called IPEC people with diabetes, brittle type one diabetes and have been observing them not only the insulin secretions but also that insulin dosage as well as the hypoglycemia so on.

00:05:22: So first it's being very successful...

00:05:26: These are autologous these are the person's own stem cells?

00:05:30: We had a stock of IBS cells in Kyoto University.

00:05:35: And then use that stock, we make the IPS cells not well... The human IPS cell derived other cells but they're not from their patients

00:05:47: because there's always been a problem That if you have autologous cells of course it is never going to be scalable.

00:05:52: It will also be too expensive.

00:05:54: But so using this method presumably has got potential for scale.

00:05:59: Yes Of course, we still need an anti-immunosuppressant for the long term period.

00:06:08: However... ...we are also doing research to make these iPS-derived islet cells capable from their immune systems which might be a second generation or third generations but at that end hoping that the eyelid transplantation, especially using the IPS-derived eyelid cells will sort of cure diabetes in future.

00:06:38: It's a fascinating area.

00:06:40: and finally are you seeing more people from Asia coming to EASD meeting?

00:06:46: where were lonely Japanese fifteen years ago?

00:06:50: You're one very few!

00:06:52: Yeah so I suppose many Japanese scientists or diabetes specialists are very much keen about coming to the ESD.

00:07:03: And despite of COVID-R, the number of the Japanese participants is increasing.

00:07:13: so we also expect a vice versa.

00:07:17: So many EASD colleagues will visit Japan or visit Asia to further exchange their ideas about diabetes and diabetes care.

00:07:28: Fantastic, thank you so much!

00:07:30: And actually that's one of the great strengths of EASDS – its networking and collaboration because if we're going to have successful research on diabetes it is certainly not gonna be done in one country but in the whole globe with a huge cohort of researchers all working together.
